LPA reviewed the staff, client, and facility files:įacility client files contain all required and updated forms. First aid kit was observed to be fully stocked.įacility staff provide attentive and constant supervision to all clients in care. There are motion censors around the home, and no cameras. Home has alarms on windows in client bedrooms and none on doors. Center for Discovery Emeryville is an outpatient eating disorder treatment center offering partial hospitalization and intensive outpatient therapy to patients of all genders and all ages. Fire extinguisher was available and accessible. Virtual treatment available: CFD outpatient programs can also be accessed via online telehealth services. Smoke & carbon monoxide detectors were tested and working properly in the home. Weekly menu, client rights, and emergency plan are visibly posted. There are no bodies of water, and the backyard is fenced. Outdoor premises are free of obstructions and hazards. There are no hot water signs posted anywhere. Water temperature in the mudroom measured 125.1 F. Water temperature in the kitchen sink measured 126.7 F. Knives, cleaning solutions, and medication are inaccessible to youth. The home is clean, safe, sanitary, and in good repair. Clients have individual beds, comfortable mattresses, clean linens and bedding that are all in good repair, and ample personal storage space. The home is operating within the capacity and limitations of the license. LPA toured the facility and grounds and observed the following: All six clients are present at the facility at the time of visit. Currently, the facility has in care 6 clients, mixed gender and within the ages 14-17. The facility is licensed to care for 6 ambulatory youths ages 6-17 years old. LPA toured the interior and exterior of the facility with the Facility Manager and Administrator/Program Director. LPA met with Facility Manager, Paige Bauer, and Program Director, Lindsey Hockridge. On Wednesday, Licensing Program Analyst (LPA) Mariah Hawkins visited the above facility for the purpose of conducting a Required Annual Inspection.
